A customer in Mira Mesa was having a problem starting Outlook. Each time, a message would come up stating Outlook didn't start correctly last time, and would you like to start in Safe Mode? A repair install did NOT do the trick, and it turns out the crash was related to an item Outlook was loading on startup. In Safe Mode, I disabled all the add ins... still no luck.
There is a file in the Outlook directory that stores all the toolbar info and various other settings, called outcmd.dat. I renamed outcmd.dat to outcmd.old and restarted Outlook, resetting all the toolbars and various addins in Outlook (also creating a NEW outcmd.dat with NO baggage). The default location for this file is: C:\Documents and Settings\%username%\Application Data\Microsoft\Outlook
